Responsibilities

• Provide high-quality nursing care to patients in hospice settings.
• Assess and monitor patients’ health status and respond to changes in condition.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Educate patients and families about care options, medication, and symptom management.
• Document patient care and maintain accurate records in compliance with regulations.
• Support families during the end-of-life process by offering emotional support and guidance.
• Participate in ongoing training and professional development opportunities.

Requirements

• Valid Registered Nurse (RN) license in the state of practice.
• Strong clinical assessment and communication skills.
• Experience in hospice, palliative, or end-of-life care is preferred.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a strong commitment to patient-centered care.
• Flexibility to work various shifts, including evenings and weekends.
• Basic computer skills and proficiency in electronic health record systems.
