Responsibilities
- Provide care management and service coordination for members of Lakeland Care, Inc. (LCI).
- Conduct comprehensive assessments to identify members’ needs and desired outcomes.
- Develop and implement member-centered care plans in collaboration with the interdisciplinary team (IDT).
- Coordinate the provision of services and supports according to established LCI policies and DHS contract requirements.
- Monitor the delivery and effectiveness of services, making adjustments as necessary to meet members’ needs.
- Serve as a primary point of contact for members and their families, ensuring effective communication throughout the care process.
- Participate in regular team meetings to discuss member progress and make collaborative decisions.
- Ensure compliance with all regulatory requirements and uphold high-quality standards of care.
Requirements
- Current RN licensure in the state of Wisconsin.
- Minimum of two years of nursing experience, preferably in care management or community-based settings.
- Strong understanding of the needs of frail elders and adults with disabilities.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work both independently and as part of an interdisciplinary team.
- Proficient in using electronic health record (EHR) systems and other relevant technology.
- Valid driver's license and willingness to travel within the service region.
