Responsibilities

• Provide care management and service coordination for members of Lakeland Care, Inc. (LCI).
• Conduct comprehensive assessments to identify members’ needs and desired outcomes.
• Develop and implement member-centered care plans in collaboration with the interdisciplinary team (IDT).
• Coordinate the provision of services and supports according to established LCI policies and DHS contract requirements.
• Monitor the delivery and effectiveness of services, making adjustments as necessary to meet members’ needs.
• Serve as a primary point of contact for members and their families, ensuring effective communication throughout the care process.
• Participate in regular team meetings to discuss member progress and make collaborative decisions.
• Ensure compliance with all regulatory requirements and uphold high-quality standards of care.

Requirements

• Current RN licensure in the state of Wisconsin.
• Minimum of two years of nursing experience, preferably in care management or community-based settings.
• Strong understanding of the needs of frail elders and adults with disabilities.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work both independently and as part of an interdisciplinary team.
• Proficient in using electronic health record (EHR) systems and other relevant technology.
• Valid driver's license and willingness to travel within the service region.
