Responsibilities
- Provide brief, evidence-based psychotherapies and supportive counseling to patients in a primary care clinic setting via telehealth.
- Conduct psychosocial assessments to identify eligible patients based on defined criteria.
- Develop, implement, and monitor treatment plans in collaboration with patients to achieve their individual goals.
- Ensure a structured discharge plan is in place at the conclusion of care.
- Deliver crisis support to patients with urgent behavioral health needs, ensuring timely intervention.
- Collaborate with primary care providers to ensure seamless integration of behavioral health services.
- Maintain accurate and timely documentation in accordance with legal and ethical standards.
Requirements
- Master's degree in Social Work (LCSW), Counseling (LPC), or Psychology from an accredited institution.
- Current and valid state licensure as an LCSW, LPC, or Psychologist.
- Experience in providing psychotherapy and counseling in a clinical setting, preferably in a telehealth environment.
- Strong understanding of evidence-based treatment modalities.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients.
- Ability to work independently and manage time effectively in a remote setting.
- Proficiency in using telehealth technology and electronic health record systems.
