Job Responsibilities
- Provide leadership and mentorship within the multidisciplinary team of the Pregnancy Medical Home program.
- Deliver comprehensive medical and social support services to pregnant and postpartum patients.
- Facilitate the transition of care for Medicaid patients from prenatal to interpregnancy stages.
- Implement and evaluate innovative care models aimed at improving patient care quality and health outcomes for mothers and newborns.
- Collaborate with various healthcare professionals to enhance service delivery in the OB-GYN department.
- Participate actively in educational and training initiatives for medical students and residents.
- Contribute to research efforts that promote advancements in maternal health.
Requirements
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current Licensed Clinical Social Worker (LCSW) certification.
- Minimum of one (1) year of relevant experience in clinical social work, preferably in a healthcare setting.
- Strong interpersonal and communication skills, with the ability to work effectively in a team environment.
- Commitment to enhancing maternal and child health through innovative care practices.
- Proficiency in using electronic health records and other relevant computer software.
