Responsibilities
- Provide brief, evidence-based psychotherapies and supportive counseling to patients in a primary care clinic setting.
- Conduct psychosocial assessments to identify eligible patients based on defined criteria.
- Develop and execute treatment plans in collaboration with patients to achieve their therapeutic goals.
- Deliver care in a remote, telehealth format, ensuring accessibility for patients.
- Implement effective discharge plans at the conclusion of therapy, ensuring continuity of care.
- Offer crisis support to patients identified as requiring immediate behavioral health assistance.
- Work collaboratively with a multidisciplinary team to enhance patient care and services.
- Maintain accurate and timely documentation of patient interactions and treatment plans.
Requirements
- Master’s degree in Social Work (LCSW), Professional Counseling (LPC), or Psychology (Ph.D./Psy.D.) is required.
- Active and unrestricted state licensure in the relevant field.
- Experience in delivering telehealth services and familiarity with technology platforms used for virtual care.
- Strong knowledge of evidence-based psychotherapy techniques.
- Excellent communication and interpersonal skills.
- Ability to manage crisis situations effectively and provide immediate support.
- Commitment to patient-centered care and the ability to work collaboratively within a team.
