Job Responsibilities
- Provide comprehensive social and medical support to pregnant and postpartum mothers within the Pregnancy Medical Home program.
- Collaborate with a multidisciplinary team to ensure the delivery of high-quality prenatal care.
- Facilitate the transition of care for patients post-delivery to interpregnancy care, particularly for the Medicaid population.
- Implement strategies aimed at improving maternal and newborn health outcomes.
- Participate actively in ongoing training and development within the department to enhance team performance and patient care.
- Engage in community outreach programs to educate and support expectant mothers and families.
- Document and monitor patient progress and outcomes, ensuring adherence to clinical best practices.
- Provide mentorship and training for junior staff members and students within the program.
Requirements
- Master's degree in Social Work (MSW) from an accredited institution.
- Current Licensed Clinical Social Worker (LCSW) certification.
- Minimum of one (1) year of relevant experience in a healthcare setting, preferably in maternal and child health.
- Strong interpersonal and communication skills to effectively interact with patients and team members.
- Ability to work collaboratively in a fast-paced, multidisciplinary environment.
- Knowledge of Medicaid policies and practices related to maternal and infant healthcare is a plus.
