Responsibilities

• Develop, assess, and facilitate care management activities for members with mental and behavioral health needs.
• Conduct evaluations of members through phone or in-home visits to identify available resources.
• Create personalized care plans that address members' mental health and substance use disorder needs.
• Educate members and their families about mental health resources and treatment options.
• Coordinate and collaborate with healthcare providers to ensure effective treatment and support.
• Track and report on care management activities to measure healthcare outcomes.
• Provide ongoing support and follow-up to ensure compliance with care plans.
• Travel up to 75% of the time within the assigned region for home and community visits.

Requirements

• Bachelor's or Master's degree in Social Work, Nursing, Psychology, or a related field.
• Proven experience in healthcare, mental health, or case management.
• Knowledge of mental health and substance use disorders.
• Strong communication and interpersonal skills for effective member interaction.
• Ability to work independently and manage time effectively.
• Willingness to travel extensively within the designated area.
• Familiarity with healthcare regulations and policies is a plus.
