Responsibilities
- Develop, assess, and facilitate care management activities for members with mental and behavioral health needs.
- Conduct evaluations of members through phone or in-home visits to identify available resources.
- Create personalized care plans that address members' mental health and substance use disorder needs.
- Educate members and their families about mental health resources and treatment options.
- Coordinate and collaborate with healthcare providers to ensure effective treatment and support.
- Track and report on care management activities to measure healthcare outcomes.
- Provide ongoing support and follow-up to ensure compliance with care plans.
- Travel up to 75% of the time within the assigned region for home and community visits.
Requirements
- Bachelor's or Master's degree in Social Work, Nursing, Psychology, or a related field.
- Proven experience in healthcare, mental health, or case management.
- Knowledge of mental health and substance use disorders.
- Strong communication and interpersonal skills for effective member interaction.
- Ability to work independently and manage time effectively.
- Willingness to travel extensively within the designated area.
- Familiarity with healthcare regulations and policies is a plus.
